Green Sleeve Advance Care Planning Workshop: Gambier Island

Green Sleeve Advance Care Planning Workshop

An accident or medical crisis can happen to anyone of any age at any time. In this workshop, presenters from Sunshine Coast Hospice will introduce you to the Sunshine Coast Green Sleeve. Think of the Green Sleeve as your healthcare passport. It’s a place to store your medical information and advance care plan (ACP) documents in case of a medical emergency or a decline in your health.

Developing your advance care plan is important for communicating your wishes for future care. Putting a plan in place will help ensure you get the care you want and help your family at a difficult time.

The advanced care planning documented in your Green Sleeve ensures you receive the level of care you want to receive if you get very sick and cannot speak for yourself.

Hosted at the Gambier Community Centre on Saturday, May 11, 1-3 pm. Seating is limited - sign up for this free workshop - call 604-885-3513.

We are grateful to the London Drugs Foundations for their funding support of this workshop.

Since its startup in 2020, more than 2,500 Green Sleeve kits have been distributed on the Sunshine Coast.

The Green Sleeve Initiative

Think of the Green Sleeve as your healthcare passport. It’s a place to store your medical information and advance care plan (ACP) documents in case of a medical emergency or a decline in your health.

Developing your advance care plan is an important process for communicating your wishes for future care. Putting a plan in place will help ensure you get the care you want, and it will help your family at a difficult time.

Bring your Green Sleeve to medical appointments or your trip to the hospital. Store it safely at home on or near the fridge, where paramedics and family know to find it during an emergency. Your Green Sleeve will speak for you if you are too ill or injured to speak for yourself.

If you would like the Coast Hospice ACP Team to present a workshop to your organization, or would like to be put on the waiting list for the next public workshop, please contact us at the link below.

Being There for Grieving Children

A safe space will be created for open conversation about supporting children and families who are grieving a significant loss. Dr Heather Mohan will reflect on some lessons she has learned in her work as a grief counsellor and expressive arts therapist, broadening our understanding of the grieving process for children, youth and families, and expanding our knowledge of effective ways to support them.

Following Heather's presentation, several members of our community who have experienced grief and loss with children will join in a facilitated conversation. Audience members will also have opportunity to ask questions and share their own experience.
